Short Form Questionnaire
Recently, several callers to my office asked that I offer an alternative to my comprehensive 35 page questionnaire. In response, I created a simple, 2 page questionnaire that should take you about 10 minutes to complete. This worksheet should give me enough information to speak with you intelligently about your debt situation and will allow us to decide how to move forward. Long Form Questionnaire:

2) make me copies of pay stubs for the past seven months. I need copies of pay stubs from you and any other member of your household who works, regardless of whether you intend to file individually or jointly. The Bankruptcy Code requires that I look at household income.
3) make me a copy of your 2009 income tax return
4) make me a copy of your Social Security card and driver’s license. When you file a bankruptcy, you will be attending a hearing called a “meeting of creditors.” At that meeting you will be required to show two (2) separate forms of ID. I need to have copies of these two pieces of identification in my file prior to filing. If you have lost your Social Security card, you will need to get a replacement Social Security card.
5) if possible, get me copies of your credit reports from all 3 credit bureaus. In many cases you can get your credit reports online at no cost from AnnualCreditReport.com. Otherwise, here are sample letters you can use to request credit reports from Equifax, Experian and Trans Union.
6) go by CarMax and get a written offer to buy. This does not mean that you are going to sell your vehicle. CarMax will give you a free written appraisal that will help us in the bankruptcy process.
7) if you have a house, call a local real estate agent (such as Re-Max or Century 21) and ask for an informal “drive by” appraisal and information about comparables in your neighborhood.
8) you will need to get your pre-bankruptcy credit counseling certificate. Here are a few vendors that you can use:
